An oblong aluminum device with a black plastic front panel. There are several status indicators and a power button on the front panel. It is branded 'supraFAXmodem 144 LC'.

I found this at a local thrift store. It requires a 9-volt AC power supply—it will turn on with 9-volt DC, but it won't function correctly. Since I don't have it's original adapter, I'm using an original NES power adapter. It won't respond to a serial port faster than 38400 baud. It's currently connected to the Apple IIC. I have been able to dial out, but it doesn't seem to receive any data after connecting. I don't know if it's a modem issue or something between the Apple II and Linux on the Pi Mini Server.
